Working as part of a team providing essential care and support to young adult and the elderly with medical, surgical, neurological, mental health disease conditions, as well as emergency accident cases, challenging behavior and related behavioral difficulties, for a big hospital hospital based in Kumasi. Carrying out everyday duties in our hospital emphasis on promoting and maintaining their well-being and independence. Participating in appropriate training activities, assisting the team , and providing individualized care at all times.


Assessed, planed and implemented and evaluated nursing care in conjunction with the multidisciplinary team. Supervised the implementation of nursing care plan. Participate and supervised delivery of evidence based care and ensured consistency and continuity of the quality of care. Seized the opportunity to be student preceptor which enabled me to mentor many student nurses. Dressed all kind of wounds aseptically using aseptic techniques. Perform advanced nursing procedures such as suturing of minor wounds, administration of oxygen, NG tube feeding as well as venipuncture, and cannulation. Educated patients on nutrition and healthy lifestyle as well as prevention, and management of their disease conditions. Administered various formulas of medications using the National drug guid and monitored patients for adverse reactions. Assisted patients in performing activities of daily living such as oral care, hands and feet’s care, complete or assisted bed bathing, and serving of bedpans. Provide emotional support and comfort to patients and maintained their safety and confidentiality. Monitored, recorded and reported changes in patient’s health status. Working as part of a team providing essential care and support to young adult and the elderly with dementia, mental health issues, neurological disorders, challenging behavior and related behavioral difficulties, for a big hospital hospital based in Kumasi. Carrying out everyday duties in our hospital and client’s home with emphasis on promoting and maintaining their well-being and independence. Participating in appropriate training activities, assisting the team , and providing individualized care at all times.

Lead the care team in assisting vulnerable adult with getting in and out of bed, bathing, oral care, incontinence and getting dressed. Record vital signs and supervise patient’s health and overall wellbeing. Support with nutritional needs, including non-oral nutrition and fluid intake, ensure that patients who require assistance at meal times were assisted with eating and drinking including monitoring the use of sharp cutlery and hot pans etc. Promptly administer oral and topical prescribed medications. Accompany patients to appointments, trips and days out and provide full support during all activities.Accomplish weekly shopping, administrative tasks, housekeeping and laundry, kept patient living space clean, comfortable and free of hazards. Arrive promptly to each patient’s home and made friendly chats to ensure comfort throughout each home visit. Provide essential emotional and physical support development needs , comfort and companionship. Make the best use of aids provide to help patients with limited mobility, or physical and learning difficulties.Adopt holistic, person centered care and by promoting maximum independence, dignity and privacy, and support them in a manner that reflect their personalities and our heart values, ensure they always make their own choices and wishes. Utilize different outstanding communication styles when increasing with patients to ensure optimal expression and satisfaction, and build positive relationships. Execute care plan and enable patient’s with intellectual disabilities, more complex needs and challenging behavior to live more independently and pursue their interest. Ensure that patients with dementia and mental health issues gain utmost satisfaction through displaying kindness, compassion, empathy, honesty, and recovery focused support.

Record and document any changes or sign of relapse with my patient, reporting it to the charge nurse immediately. Assure safety of patients by reviewing treatment plans and conducting risk assessment, organize best social event to entertain them. Identify stimulating activities like story- telling, crafts and fun games that improve their engagement. Streamline the reporting system for efficient time management/records keeping inspection, saving over an hour a day. Liaise with other health and social care professionals to provide the best possible care. Design work schedules, task and rosters for junior team members and offer training and support to ensure compassionate care. Ensuring communication of a personal and or private nature is discrete and patient confidentiality is maintained. Uphold Hospital standards at all times.
