· · Performs head-to-toe assessment on all clients.

· Fall risk assessment/prevention

· Monitors the hemodynamic status of patient and correctly interpret the results

· Administers medication via oral, nasal, intravenous, rectal, intramuscular and subcutaneous route.

· IV insertion and lab draws for patients.

· Foley catheter insertion – male and female patients.

· Prepares the patient prior to X-ray, ultrasound, CT scan, CT angiography, MRI, echo, EEG and other diagnostic procedures.

· Wound care assessment and management specific to the wound type: diabetic foot, abscess, NGT site, Central line insertion site, pressure sores and ensuring wound cultures are taken.

· NGT insertion and PEG care, NGT and PEG tube feeding.

· Adequately assesses for pain and utilizes appropriate pain management techniques.

· Oxygen administration via nasal cannula, face mask, non-rebreather mask, rebreather mask, and bag valve mask ventilation.

· Diagnostic studies such as blood glucose taking, urine, stool specimen collection, urine dipstick, ketone strip and pH strip analysis.

· Designs a nursing care plan for each client and evaluating its efficacy by documenting outcomes.

· Demonstrates knowledge in cardiac monitoring identifies dysrhythmia and conducts treatment appropriately.

· Provides support and adequate education to the patient and family, including them in formulating plan of care.

· Performs assessment on patients in each shift and assign staff base on competency level when being shift in charge.

• Assess patients' health status through physical exams and reviewing medical history, including allergies, medications, and surgical risks.
• Provide detailed information to patients and their families about the surgery, what to expect, and how to prepare, including fasting, medications, and postoperative care.
• Administering medications, ensuring that patients have fasted appropriately, conducting skin preparation, and securing consent for surgery.
• Offering emotional support to patients who may be anxious about their upcoming surgery, providing reassurance, and answering questions.
• Continuously monitor vital signs like blood pressure, heart rate, respiratory rate, and oxygen levels to detect any signs of complications.
• Administer prescribed pain medications and assess their effectiveness, as well as provide non-pharmacological pain relief techniques, are essential parts of postoperative care.
• Inspect, clean, and dress surgical wounds, ensuring they are healing properly and showing no signs of infection.
• Help patients with early mobilization to prevent complications like deep vein thrombosis (DVT), improve circulation, and overall recovery.
• Assist with discharge planning, educating patients and caregivers about home care, wound management, medication schedules, and follow-up appointments.

• Assess the patient’s physical condition, including vital signs, allergies, and medical history, ensuring that all necessary information is documented before surgery.
• Ensuring that Operating Room is properly prepared, with sterile surgical instruments, supplies, and equipment arranged and ready for use.
• Verifying the patient's identity, surgical procedure, and site, ensuring informed consent has been obtained.
• Ensure that any pre-surgical tests (such as blood work or imaging) are completed and reviewed.
• Assist in positioning the patient on the operating table, ensuring proper alignment to prevent injury during the surgery.
• Provide emotional support to the patient and ensure their comfort.
• Maintain a sterile field throughout the procedure, ensuring that sterile techniques are followed.
• Responsible for scrubbing in, donning sterile gloves and gown, and passing instruments to the surgeon in a sterile manner.
• Prepare and manage surgical instruments, counting them before and after the procedure to ensure none are left inside the patient.
• Hand instruments, sutures, and other materials to the surgeon and assistants during surgery.
• Anticipate the surgeon’s needs by knowing the surgical procedure in detail and preparing instruments and supplies in advance.
• Monitor the sterile field to ensure that no one or nothing compromises its sterility during the operation.
• Act as the patient’s advocate during surgery to ensure their safety and addressing any needs during the procedure.
• Act as liaison between the sterile surgical team and non-sterile areas of the operating room.
• Oversee the overall environment of the OR, including maintaining proper lighting, room temperature, and equipment function.
• Document the surgical procedure, including patient information, the start and end times, medications administered, instruments used, and any other relevant details.
• Responsible for properly labeling and sending specimens (e.g., tissue biopsies) to the pathology department.
• Nurses assist with the safe transfer of the patient from the operating table to a stretcher or bed, ensuring the patient is moved carefully and with all necessary equipment (e.g., oxygen, IV lines) in place.
• Complete documentation on the patient's status post-surgery, ensuring that any surgical notes, instrument counts, and patient information are accurately recorded.
• Assist in the immediate postoperative phase, ensuring the patient is stable and that any urgent needs (e.g., pain management, wound care) are addressed.
• Responsible for maintaining strict sterile techniques to prevent infection, including proper hand hygiene, sterilization of instruments, and ensuring the OR environment is free from contaminants.
• Monitor the team’s compliance with aseptic practices, such as wearing sterile gloves, gowns, and masks, and ensuring proper disposal of waste and used materials.
• Ensuring patient safety, ensuring that protocols such as "time-out" (a pause before surgery to verify the correct patient, procedure, and site) are followed.
• Administer medications, such as antibiotics, and other necessary drugs, ensuring the correct dosages are given at the right times during surgery.
• Preventing surgical errors by confirming patient identity, marking the correct surgical site, and ensuring that all team members are aware of the planned procedure.
• Work closely with surgeons, anesthesiologists, and surgical technologists to ensure that the surgery proceeds smoothly.
