Key Duties and Responsibilities

• Patient Assessment: Conducted comprehensive patient assessments, developed care plans, implemented and evaluated patient outcome.
• Personal and Bedside Care : Assisted with activities of daily living (ADLs) like bathing, dressing, and toileting.
• Medication Manager: Helped with medication reminders and administration.
• Meal Preparation: Prepares and serves meals according to dietary needs.
• Patient Safety and Risk Management: Identify and address potential safety risks and maintains infection prevention control measures

Key Responsibilities and Duties

• Medical Care: Performed medical procedures such as vital sings monitoring, inserting intravenous lines, venipuncture, administering medications
• Safety :Ensured clients' safety and well-being through attentive care and companionship.
• Transportation Assistant: Assisted with transportation to appointments and errands for clients
• Care plan Implementation: Assessed, planned, implemented, and evaluated individualized care plans for patients
• Documentation and record keeping: Maintained accurate and up-to-date records of patient information, ensured confidentiality and adhered to data protection regulations
