• Monitor vital signs daily and intervene in any deviations in vitals.

Clinical duties

• Providing guidance on health maintenance and disease prevention to patients and the general public
• Give bed bath to very ill patients and prevention of pressure sores
• Maintain patients personal hygiene
• Take, chart vital signs and serve medication
• Admitting, discharge and document patients data
• Plan and implement patients care
• Serve and feed patients
• Care for dead and manage bereavement
• Carry out patients teaching and write reports
• Organise and conduct home visit
• Organise and conduct health promotions programmes
• Counsel patients and relatives

• Assisting patient with hygiene, physical comfort, eating and drinking and mobility while observing and reporting specific changes.
• Regular monitoring of patient's condition, assess their wounds, provided wound care to promote wound healing and prevent infection
• Reports and communicate to supervisors on any deviations
• Raising safeguarding concerns and assisting patient to take decisions helpful to their health and recovery
• Developing and improving the healthcare plan in collaboration with other healthcare personnel in the household
• Providing palliative care as needed to keep the patient comfortable
• Make recommendations for devices and tools that might improve the patient’s quality of life
• Documented patient progress and care interventions in medical records.
• Reduced hospital readmissions by providing thorough discharge education and follow-up support to patients.
